The case

A 25-year-old man presents with familial dilated cardiomyopathy. He is on maximally tolerated medical therapy. His left ventricular ejection fraction is 25% and he remains in NYHA class II. He had a CRT-D device implanted 4 weeks ago, and is referred for comprehensive cardiac rehabilitation.
