Dr. Lydia Tran, PharmD, BCPS is the Cardiovascular Senior Clinical Pharmacy Specialist for the System Heart & Vascular Clinical Services at Yale New Haven Health. She graduated from University of Texas at Austin (B.S. Neuroscience), Doctor of pharmacy from University of Saint Joseph & residency at Yale New Haven Hospital. She serves as the Clinical Advisor to Anticoagulation & Cardiology Subcommittee, pharmacy lead on HF Transitions Subgroup & HVC Care Pathway Clinical Consensus Council. She is board certified, Adjunct Assistant Professor at University of Conn & Lecturer at Yale School of Medicine PA Program on cardiology topics. She serves on the Executive Board of CT Pharmacists Association, ACCP Cardiology Programming Committee, ACC Education Committee. Dr. Tran is the 2018 recipient of the Cardinal Health RX Champion Award for her integral role in the development of the CT Naloxone Training and Certification & the 2016 recipient of ASHP Exceptional Service and Leadership Award.
