To determine how to address the cardiotoxicity of acute hyperglycemia in a significant number of acute coronary syndrome patients. Should we aggressively treat the hyperglycemia to reduce pro-arrhythmicity or should we target the pathways involved in glucose-induced arrhythmicity, given the significant risks associated with hyperglycemia? Those risks involve electrical disturbances in myocardial repolarisation, which become particularly acute when coupled with ischaemic events where dysregulation of intracellular calcium further exacerbates repolarisation dysfunction. Furthermore, cardioprotective interventions often fail in hyperglycemic patients and animal models.
